Parish Census Form To become a registered parishioner of St. John please print out this form, fill out all information paying special attention to the key at the bottom of the page. Also please note the area under the Head of House/Spouse section that asks for information about your marriage. This section often gets overlooked. E-mail to DPerque@eatel.net
Stewardship of Ministry Form This form is used to sign up for the various ministries in the parish. This form should be returned to the parish office. This form enables us to draw from a computer database as needed. E-mail to DPerque@eatel.net
Offertory Direct Debit Form This form is used to have your weekly or monthly offering debited from your checking or savings account. Please follow directions on the form and return to Karen Hagendorfer in the parish office. Khagendorfer@eatel.net
Baptism Guidelines This document details what is necessary for Baptism in this parish as well as giving the dates of Baptism seminars and scheduled Baptism dates.
Children's Ministry Forms
Nursery Registration Form This form is used to register children newborn to 3 years of age for the nursery during the 9:30 and 11:30 Sunday Masses. This is a co-op Nursery. everyone who utilizes the nursery must sign up to work in the nursery.
Christian Formation Service Hours Form This form is for students in grades 9-12 to record service hours. Forms are to be turned in to Mr. Shows upon completion.
Confirmation Sponsor Eligibility Form This form is to be downloaded and filled out by the Confirmation candidates sponsor. It is to be signed by their pastor and returned to the Christian Formation office.
Saint Name Form This form is to be used by the Confirmation candidate. The candidate should research the saints and choose one that best suits them.
Requirements for volunteering to work with Children in the Diocese of Baton Rouge.
The following links all lead to the Diocese of Baton Rouge Child Protection website. All three headings below are listed on the left hand side of the OCYP homepage. The three steps below must be completed in order to work with children in this diocese. Completed documents should be dropped off, mailed, scanned in or emailed to Lisababin@eatel.net
1. EAPPS is the background check application it must be completely filled out with names, addresses, dates etc. use N/A in blanks that do not pertain to you.
2. Code of Ethics and Behavior -The Code of Ethics must be read in its entirety. Sign the last page and turn in with application. No application will be processed until this has been turned in.
3. Safe and Sacred Online Training You must create a username and log-in on your first visit. This is a short online course and quiz with a printable certificate when completed. Certificate MUST be turned in with EAPPS and Code of Ethics forms.
